Since March of 2008, Rocking Horse Community Health Center has been privileged to provide a medical home with quality comprehensive care to adults. Meeting the needs of each person regardless of their financial circumstances and helping to improve the availability and accessibility of essential primary and preventative healthcare is our constant goal.
Primary and chronic health care for adults provided by Rocking Horse includes scheduling for an Initial Evaluation, Follow-up Appointments, Diabetic Education Chronic Health Management, Specialty Visit Referrals, Medical Prescription Assistance Program and Mental Health Screenings.
